• Deliver skilled nursing care to patients in their homes, following physician orders and individualized care plans.\n
• Conduct initial assessments and collaborate with Population Health Case Managers.\n
• Supervise and support LPNs and Home Health Aides.\n
• Focus on achieving excellent patient outcomes and satisfaction.\nWhat You'll Need\n
• Graduate of an accredited nursing program.\n
• Active RN license in Delaware.\n
• Minimum 1 year of nursing experience (Home Care or Acute Care preferred).\n
• Valid driver's license and reliable transportation.\n
• Willingness to work 1 weekend / month and 2 holidays / year (1 summer, 1 winter).\nBenefits Starting Day One\n
