An established and mission-driven Federally Qualified Health Center (FQHC) is seeking Board Certified or Board Eligible Internal Medicine or Family Practice Physicians to join its dedicated clinical team in downtown Los Angeles. This role offers schedule flexibility and a unique opportunity to work with underserved and homeless populations in an outpatient setting.
Key Responsibilities
- Provide comprehensive outpatient medical care, including acute, chronic, and preventive services.
- Work with a dedicated care team to serve as a patient-centered medical home for adult patients (18+).
- Perform in-office procedures : incision and drainage, arthrocentesis, nail removal, skin biopsy, cryotherapy, etc.
- Maintain accurate and timely documentation; progress notes must be locked within 7 calendar days.
- Review and sign off on labs, diagnostics, and consult notes; follow up with patients as clinically appropriate.
- Make and manage referrals, including e-consults, to specialty care providers.
- Participate in morning huddles, provider meetings, and all-staff meetings.
- Provide compassionate care to patients who may have complex social, mental health, and substance use challenges.
- Participate in Continuous Quality Improvement (CQI) initiatives and peer reviews.
- Optionally obtain MAT waiver to assist in treatment of substance use disorders.
- Supervise Advanced Practice Providers (APPs) as needed; initial and ongoing chart review responsibilities may apply.
- Collaborate with medical assistants and administrative teams to ensure quality, coordinated care.
- Occasionally support community outreach health screenings.
- Maintain active enrollment in Medicare, Medicaid, Managed Care plans, and complete required credentialing documentation.
