The On-Call RN provides after-hours hospice nursing support through home and facility visits, patient / caregiver education, and phone triage. This role ensures patients and families receive timely, professional, and compassionate care during evenings, nights, weekends, and holidays.

Key Responsibilities :

• Provide on-call coverage as scheduled, including patient visits and phone triage.
• Assess and respond to urgent patient / caregiver needs, including symptom management and emotional support.
• Collaborate with attending physicians, hospice staff, and caregivers to ensure coordinated and optimal care.
• Educate patients and caregivers about hospice philosophy, disease progression, and plan of care.
• Communicate patient status changes and shift reports to the Interdisciplinary Team (IDT).
• Support RN Case Managers with scheduled visits and after-hours needs.
• Demonstrate adherence to hospice philosophy, professional standards, and organizational values.

Qualifications :

Schedule : Monday - Sunday 5 : 00pm - 8 : 30 am - 7days on / 7days off
