Licensure : Active Indiana license or IMLC

Board Status : BC / BE in Hematology / Oncology (within 5 years of residency completion)

Experience : 2+ years preferred

Certifications : BLS and ACLS (AHA issued)

Vaccinations : Flu vaccine required (medical / religious exemptions accepted)
