Position Overview :We are seeking a skilled Advanced Nurse Practitioner to join our team. We are a partner with Holy Name Medical Center and continue to grow our footprint in Northern New Jersey.
Responsibilities
Manage both acute and chronic conditions , with a focus on continuity of care across rehab, long-term care, and outpatient settings.Provide primary care services to patients across the lifespan in a variety of settings, including outpatient offices, rehabilitation centers, and nursing homes.Perform comprehensive physical examinations and obtain detailed medical histories .Order and interpret diagnostic tests , including laboratory work and medical imaging, to support accurate clinical decisions.Prescribe and manage medications and treatments within the NP scope of practice.Conduct health coaching and patient education , promoting wellness, preventive care, and chronic disease management.Collaborate closely with physicians, therapists, nursing staff, and other healthcare professionals to develop and implement coordinated patient care plans.Provide specialized follow-up care in rehab and nursing home populations, addressing issues such as mobility, pain management, medication compliance, and discharge planning.Utilize electronic health record systems (e.g., eClinicalWorks) to accurately document encounters, coordinate care, and track patient progress.Maintain high standards of infection control and aseptic technique in all procedures and care settings.Support transitions of care by communicating effectively between rehab / nursing facilities and outpatient office staff.Qualifications :
Valid Nurse Practitioner license in the state of NJMaster's degree in Nursing with a specialization as a Nurse PractitionerCertification as a Nurse Practitioner in the appropriate specialty area (e.g., Family, Pediatrics, Adult-Gerontology)Experience in primary care or a related field preferredStrong clinical skills and knowledge of evidence-based practice guidelinesExcellent communication and interpersonal skills to effectively interact with patients, families, and healthcare team membersPlease note that additional qualifications may be required based on the specific needs of the healthcare facility.
