Job description

Job Summary :

The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. This role involves patient assessments medication administration treatment planning and collaboration with an interdisciplinary team. Strong communication and collaboration skills are essential for working with patients providers and team members.

Location : Mission Hills California United States

Responsibilities :

  • Provide skilled nursing care to patients.
  • Conduct patient assessments.
  • Administer medications.
  • Develop and implement treatment plans.
  • Collaborate with the interdisciplinary team.
  • Provide case management services.
  • Deliver hospice care.
  • Maintain accurate patient records.

Required Skills & Certifications :

  • Active Registered Nurse license in the state of working.
  • Bachelors degree in Nursing or related field.
  • Clinical Assessment skills.
  • Medication Administration skills.
  • Treatment Planning skills.
  • Interdisciplinary Team Collaboration skills.
  • Minimum of 2 years of hospital home health or hospice care experience.
  • Drivers license car insurance and automobile for field work.

  • Specialities in Medical / Surgical ICU ER Psychiatric Labor & Delivery PACU Operating Room.

    • Special Considerations :

  • Acute Care Hospital Magnet Facility.
  • Scrub Color : Any color (L / D & OR provided).
