About The Position

Job Title : Physical Therapist

Hours per Shift : 8 : 00 AM - 4 : 30 PM

Experience : Minimum of one year experience in Long Term Care / Home Care setting as Physical Therapy

License : Current Client License in Physical Therapy

Certifications : BLS (AHA ONLY)

Must-Have :

• Home Health Experience within the last year
• 1 year of PT experience in a home health setting

Description :

Provides case management and direct patient care within a team environment.

Administers comprehensive physical therapy modalities in the home setting.

Assists in evaluating functional levels and developing care plans.

Prepares clinical and progress notes as required.

Consults with family and other personnel regarding patient care.

Participates in in-service programs for continuous learning.

Promotes effective working relationships and teamwork.

Utilizes resources efficiently to contain costs and enhance service value.

Communicates positively and respectfully with all stakeholders.

Supports public relations and community outreach initiatives.

Strives for continuous improvement in physical therapy functions.

Develops a customer-oriented environment responsive to patient needs.

Initiates appropriate rehabilitative programs and conducts evaluations.

Provides guidance and instruction to patients and families.

Implements home therapy programs tailored to patient needs.

Supervises para-professional staff involved in patient care.

Participates in patient conferences to ensure coordinated treatment plans.

Assists in the orientation of new rehabilitation personnel.

Completes necessary clinical records in compliance with regulations.

Maintains current knowledge of available community services.

Ensures patient needs are met throughout the care delivery process.

Monitors quality indicators and customer satisfaction.

Contributes to professional growth and development in the workplace.

Participates in continuing education and shares knowledge with the team.

Demonstrates commitment to quality and compliant care practices.

Recognizes the importance of positive interactions with patients and staff.
