Job Description
We are looking for an enthusiastic and compassionate Psychiatric Mental Health Nurse Practitioner (PMHNP) to become part of our clinical team. We are a trusted provider of comprehensive behavioral health services for residents in nursing homes throughout Plattsburgh, NY . This role is ideal for a practitioner who is deeply committed to geriatric mental health and motivated to enhance the emotional and psychological well-being of older adults.
Requirements
Position Responsibilities
Perform detailed psychiatric assessments and evaluations for residents in long-term care settings.
Develop and implement individualized treatment plans that reflect each patient’s needs and diagnoses.
Oversee medication management, including monitoring progress and adjusting treatments as clinically appropriate.
Coordinate care with interdisciplinary teams such as physicians, psychologists, social workers, and nursing staff to ensure a fully integrated approach.
Provide supportive counseling and psychotherapy to patients and, when appropriate, their families.
Educate residents and caregivers on mental health diagnoses, medication options, and long-term wellness strategies.
Maintain thorough, timely, and compliant documentation within the EHR system.
Participate in case reviews, team meetings, and initiatives focused on quality improvement and best practices.
Required Qualifications
Active New York Nurse Practitioner license with certification as a PMHNP .
Ability to independently manage a diverse patient caseload with strong clinical judgment.
Proven competence in psychiatric diagnosis, treatment planning, and psychopharmacology.
Strong interpersonal and communication abilities, with a focus on patient rapport and collaborative teamwork.
Proficient in electronic health records and general computer usage.
