• Physician MD / DO credentialed from the Tennessee Board of Medical Examiners obtained prior to hire date or job transfer date. KY License also required if working in a Kentucky facility. Education :
• Doctor of Medicine (MD) or Doctor of Osteopathy (DO) required.
