Care Coach II

A company is looking for a Care Coach II to support family caregivers through technology and proven care methods.

Key Responsibilities

Engage and educate caregivers through phone and digital technology to improve their well-being

Manage case assignments and assess caregiver and care recipient needs to create care plan goals

Document coaching activities and collaborate with stakeholders to identify solutions for caregiver engagement

Required Qualifications

Bachelor's Degree with 4-6 years of experience in care management or health coaching

Experience with older adults, dementia, and chronic condition management is required

Managed care or health plan experience is strongly preferred

Multilingual skills, specifically in Spanish, are required

Ability to work flexible hours, including on-call nights and weekends
