As a Physical Therapis t, you will provide services that help restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities of residents / patients suffering from injuries or disease. You will be responsible for developing treatment plans, monitoring progress, and providing guidance and support to patients throughout their rehabilitation process. What You'll Do :
Establishes and administers a treatment plan with specific therapy goals determined according to the patient's capacity and tolerance under the direction of the physician.Assesses patient needs; plans for, evaluates and modifies care to meet goals of physical therapy interventions.Collaborates with all disciplines to plan and evaluate team goals for each patient.Sets realistic and achievable goals for patients / residents.Documents and records the patients' / residents' condition and progress per state and federal regulations.Educates patients and families in an appropriate therapy method.Optimizes the functionality and skills of patients by providing, directing and / or overseeing the screening of all residents.Adhere to Minimum Data Set (MDS) procedures and effective care planning.Provides consultation and clinical supervision to PTA's working in the facility.About You :
Bachelor's, Master's or Doctorate Degree from a college or university with an accredited Physical Therapy programCurrent Physical Therapist license in WACurrent CPR certificationSix months experience in a long-term care environment preferredAbility to maintain confidentiality.Knowledge of HIPAA.General knowledge and understanding of managed care.Knowledge of state and federal rules relating to therapy delivery and documentation in skilled nursing facilities.What we Offer :
