About Position :   The primary responsibility of the Rehabilitative Services Aide is to assist therapists with treatments, conduct maintenance programs under the direction of the therapists, order durable medical equipment, perform group exercise developed by the therapists and provide administrative support to the therapists.

  • Work Location :   1214 Vaughn Rd, Burlington, NC 27217
  • Schedule : Monday through Friday, 8 : 00am to 4 : 30pm

  • Duties / Responsibilities -

  • Coordinate, under the direct supervision of the Rehab Therapist (PT, OT, ST) the necessary materials for a therapist to conduct their treatments and / or assist with transporting the participant to the treatment area.
  • Conduct the specified activities in the maintenance plan, under the direction of the therapist.
  • Document in Centricity the participant progress regarding the plan and communicate with the therapist for any out of the norm observations.
  • Assist the therapist with ordering DME when needed and follow the DME ordering workflow.
  • Enter progress notes in Centricity to document maintenance plan.
  • Assist therapist with the coordination of daily schedule of the participants for treatments and provide administrative support for the therapists.
  • Conduct the group exercise program daily, as developed by the therapist and document exercise program attendance in the group exercise log.
  • Assists therapist in coordinating the necessary materials for group exercises.
  • Communicates effectively with participants, families, staff, agencies and other departments within PHS.
  • Assists assigned participants with communication needs and other activities as assigned.
  • Participates in teaching / learning process with others.
  • Serves as an IDT member as assigned and participates in assigned team meetings.
  • Communicates all changes in participant condition to team members.
  • Works effectively in a team setting.
  • Abides by all established SeniorCare polices, rules and regulations.
  • Qualifications -

    Education :   High school diploma or equivalent required. Undergraduate degree in exercise physiology or related field preferred. One year of experience with the frail or elderly preferred.  Experience as a Rehab Technician / Assistant preferred.

    Required : Must be able to work effectively in a team environment and with geriatrics.  Must meet a standardized set of competencies for the specific position description established by PHSC and approved by CMS before working independently.

    Immunizations : All PHS Required Immunizations.  Be medically cleared for communicable diseases and have all immunizations up-to-date prior to beginning employment.

    Pay Range - $16.43 - $22.08
