Registered Nurse – Case Manager
Schedule : Monday–Friday, 8 : 00 am - 5 : 00pm
Location : Lady Lake, FL
Position Summary
The Registered Nurse (RN) plays a vital role in supporting the interdisciplinary care team by ensuring accurate documentation, coordinating care activities, and assisting with benefit determinations related to Medicare Parts A, B, and D. This role requires collaboration with providers, advanced practice clinicians (APCs), and facility staff to identify and respond to changes in patient conditions, facilitate communication, and ensure quality care delivery for patients with complex medical needs.
Primary Responsibilities
- Ensure accurate, complete, and timely clinical documentation.
- Communicate and collaborate effectively with the interdisciplinary care team.
- Assist providers and team members with various care coordination activities.
- Support benefit determinations associated with Medicare Part A, Part B, and Part D.
- Collaborate with the primary APC and facility staff to identify and respond to patient changes in condition.
- Coordinate therapy services and assist with therapy-related care plans.
- Participate in facility-based meetings, family conferences, and team case presentations.
- Coordinate training, track compliance, and ensure adherence to quality measures.
- Assist in creating and conducting in-services for selected audiences.
- Support providers in ensuring complete and accurate documentation, coding, medication reconciliation, nursing rounds, and patient assessments.
- May write verbal orders from providers in compliance with state RN practice laws and facility policies.
- Assist in managing members through acute medical situations or exacerbation of chronic conditions, including coordinating diagnostics, testing, and interventions.
- Support members during short-term sub-acute rehabilitation episodes.
Required Qualifications
Active and unrestricted RN license in the state of residence (or ability to obtain by start date).Associate degree or higher in Nursing.Current Basic Life Support (BLS) certification.Ability to perform physical requirements, including lifting up to 30 pounds and completing physical assessments.Access to reliable transportation.Full COVID-19 vaccination (in accordance with federal, state, local, and client requirements).Successful completion of a pre-employment drug screeningPreferred Qualifications
Minimum of 1 year of clinical experience; long-term care setting preferred.Experience caring for patients with complex behavioral, social, and / or functional needs.Understanding of geriatrics and chronic illness management.Ability to work effectively with diverse care teams across multiple settings.Willingness and ability to travel between facilities as needed.
