What Youll Do
Receive referrals by phone or in person for the designated departmentExplain benefits to patients, families, and community contactsAssist in finding alternative services if the patient is not a hospice candidateOversee scheduling of initial nursing assessments and admissionsMaintain admission packets and ensure proper forms are available to staffAssist community outreach with marketing activities to promote referral sourcesComplete admission paperwork as neededMaintain punctuality and attendanceProvide telephone and clerical support to staff as neededMaintain accurate clinical records, tracking client goals and progressQualifications
Be 18 years or olderAt least one year of experience in homecare, hospice, or healthcare preferredAbility to complete documentation accurately and timelyHandle stressful and delicate situationsStrong interpersonal, verbal, and written communication skillsValid drivers license and proof of insuranceAdditional Details
