Here, you will provide expert care focused on symptom management , family support , and aligning care with patient goals in both outpatient and inpatient settings.

Your Role and Responsibilities

As a key member of our interdisciplinary team, you will collaborate with physicians, APPs, social workers, and nurses to support a comprehensive palliative care program. Your responsibilities will include :

• Providing clinical oversight and guidance in both outpatient and inpatient settings.
• Leading family meetings and managing complex symptoms.
• Fostering program growth in a faith-based, patient-centered health system.
• Collaborating with the executive medical director to expand outpatient services.
