Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.
Providing compassionate end-of-life care in patient homes and residential settings, this Hospice Registered Nurse (RN) role involves managing comprehensive hospice care plans and collaborating with an interdisciplinary team to ensure patient comfort and dignity. This full-time, 13-week contract position requires strong clinical skills and empathy for patients facing terminal illness.

Schedule and Work Environment :
- Monday Friday, 8 : 00 AM 5 : 00 PM
- Occasional weekend or on-call rotation as needed
- Field-based visits to patient homes, assisted living facilities, or nursing homes
- Caseload of approximately 45 patient visits per day
- Daily travel within assigned service area with mige reimbursement
Key Responsibilities :
Conduct thorough nursing assessments and develop individualized hospice care plansDeliver skilled nursing focused on symptom and pain management to enhance patient comfortAdminister medications and treatments per physician orders and hospice protocolsMonitor patient conditions, communicate changes to the hospice medical director and care teamEducate patients and families on disease progression, medication administration, and support strategiesCollaborate with social workers, chaplains, aides, and volunteers to meet physical, emotional, and spiritual needsDocument care accurately using Home Care Home Base (HCHB)Participate in case conferences, team meetings, and quality improvement initiativesEnsure compliance with hospice policies and regulatory guidelinesQualifications :
Active RN license in good standing in the state of practiceMinimum of 1 year of recent hospice nursing experienceProficiency with Home Care Home Base (HCHB)Strong clinical assessment and communication abilitiesReliable transportation, valid drivers license, and auto insuranceCompassionate, emotionally resilient, and able to work independentlyPreferred :
