Job Summary :
The Home Health RN Case Manager is responsible for delivering high-quality nursing care in patients' homes by applying the nursing process : assessment, planning, implementation, and evaluation. This role involves providing tailored care for patients across all age groups, from adults to geriatric individuals, in accordance with established policies, procedures, and nursing standards.
Primary Job Duties :
- Conduct thorough assessments and develop, implement, and evaluate patient care plans based on individual age and diagnosis.
- Manage a diverse caseload effectively and coordinate care with a multidisciplinary team.
- Communicate essential information while adhering to agency policies and guidelines.
- Contribute to overall program effectiveness and quality of care.
- Organize and execute work tasks efficiently and effectively.
- Adjust schedules as needed to optimize agency performance.

Job Specifications :
Education : A graduate of an accredited nursing program (AD, Diploma, or BSN).Licensure / Certifications : Current RN license in the state of practice and CPR certification required.Experience : Minimum of one year RN experience; two or more years of clinical experience preferred. Home health experience is advantageous.Technical / Motor Skills : Proficient hand / eye coordination to administer injections, operate equipment, and utilize computers.Interpersonal Skills : Ability to foster collaborative relationships with patients, families, medical professionals, and staff.Physical Requirements : Capability to handle objects weighing at least 50 pounds, with frequent lifting, moving, and positioning of patients.Mental Abilities : Strong assessment and planning skills, with the ability to teach and explain patient care procedures.Sensory Requirements : Visual acuity to assess patients and interpret data, along with hearing ability for auscultation and communication.Exposure to Hazards : Possible exposure to blood-borne pathogens, infectious diseases, and environmental hazards during home visits.Hours : Flexible schedule, including weekdays, weekends, and holidays as needed to meet care obligations.Population Served : Will provide care for adolescents, adults, geriatrics, and pediatrics.License Requirements : Must possess a valid driver's license and have access to a reliable vehicle.
