Home Health RN Case Manager

Job Summary :

The Home Health RN Case Manager is responsible for delivering high-quality nursing care in patients' homes by applying the nursing process : assessment, planning, implementation, and evaluation. This role involves providing tailored care for patients across all age groups, from adults to geriatric individuals, in accordance with established policies, procedures, and nursing standards.

Primary Job Duties :

• Conduct thorough assessments and develop, implement, and evaluate patient care plans based on individual age and diagnosis.
• Manage a diverse caseload effectively and coordinate care with a multidisciplinary team.
• Communicate essential information while adhering to agency policies and guidelines.
• Contribute to overall program effectiveness and quality of care.
• Organize and execute work tasks efficiently and effectively.
• Adjust schedules as needed to optimize agency performance.
• Uphold the values of the agency in all interactions.
