The Clinical Manager (CM) is responsible for managing the delivery of quality clinical services within an assigned group of patients. The CM ensures that all assigned caregivers receive standardized, comprehensive and effective. The CM partners with the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding caregivers; Ensures the completion of a comprehensive assessment upon admission, recertification, post hospitalizations and PRN for all assigned patients
The CM will ensure training for all assigned caregivers. The training will be standardized, comprehensive and effective.
KEY RESPONSIBILITIES
Partners with the Director of Nursing (DON) and Director of Operations (DOO) in identifying, interviewing, and onboarding caregiversProvides supervision, leadership, training, guidance, and mentoring for all assigned caregiversCollaborates with the leadership teams and the compliance committee in identifying, developing, and the delivery of consistent clinical informationEvaluates assigned patient's appropriateness for homecareEnsures the completion of a comprehensive assessment upon admission, recertification, post hospitalizations and PRN for all assigned patientsEnsures a current physician order, plan of care / care plan is place for all assigned patientsManages the patient care by supervision and care coordination for all assigned patients and including the disciplinary teamEnsures that the competency program is followed and provides educational support for the caregivers as needed for skills developmentPromotes employee engagement and retentionParticipates in the review of policies and procedures to ensure compliance with all State, Federal, and Accreditation standardsParticipates in the quality assurance performance improvement (QAPI) programAssists in the coordination and oversight of all plans of correction and quality initiatives as directed by the DONPartners with the DON and DOO to lead cohesive teams toward common goalsAssists management with the development and implementation of new products and servicesMaintains professional, supportive, and responsive interpersonal communication skillsPerforms other duties as assignedROLE QUALIFICATIONS
EDUCATION & EXPERIENCE REQUIRED
RN Nursing Degree. Unencumbered RN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognizedRegistered Nurse with 1 year of clinical experienceValid driver license with proof of insuranceWorking knowledge of home care regulatory and federal requirementsProven customer service skills, sound independent judgment, initiative and solid organization and time management skills requiredStrong computer skills to include Microsoft Office SuiteAbility to travel patient homes throughout designated areaPREFERRED
1 year of supervisor experienceExcellent organization, problem-solving, decision-making, presentation, and communication skillsPossess outstanding leadership, teaching, and training skillsForward thinker with the ability to be creative and "think outside the box"WORKING CONDITIONS / EQUIPMENT USE
Work is performed indoors in a typical office environment - not substantially exposed to adverse environmental conditions.Must be able to lift up to fifteen (15) poundsFrequent use of office machines to include telephone, computer, and printerTravel may be required by car or airplane to local, out-of-town, or state seminars, conferences, meetings or other branch office locations.KEY DETAILS :
This position requires the employee to work in the office Monday through Friday during core business hours. Core business typically fall between 8 : 00 am to 5 : 00 pm with some flexibility with start and end times.
