Job Description : We are seeking a dynamic and experienced Director of Patient Care Services to join our team. The successful candidate will be a Registered Nurse (RN) with extensive home health experience. This leadership role involves overseeing the clinical operations, ensuring the highest standards of patient care, and maintaining compliance with all regulatory requirements.

Key Responsibilities :

• Lead and manage the patient care services team, including nurses, therapists, and other healthcare professionals.

• Monitor and evaluate patient care services to ensure compliance with state and federal regulations.
• Oversee the development and implementation of patient care plans.

• Collaborate with physicians, patients, and families to coordinate comprehensive patient care.
• Manage the performance improvement process to enhance patient outcomes.
• Ensure accurate and timely documentation of patient care services.
