Kentucky Licensed Care Manager

A company is looking for a Care Manager Transition of Care.

Key Responsibilities

Evaluate member needs through post-discharge assessments and medication reconciliation

Develop care / service plans in collaboration with healthcare teams to support member transitions

Facilitate communication and services between members, providers, and resources to meet medical and socio-economic needs

Required Qualifications, Training, and Education

Master's degree in Behavioral Health or Social Work, or a degree from an accredited school of nursing

2 - 4 years of related experience

Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required

Kentucky residency is required

Must possess a compact license
