Position Summary : We are seeking a dedicated and patient-focused Licensed Practical Nurse (LPN) to join our clinical team. The ideal candidate will support our providers in delivering high-quality care while ensuring an efficient, friendly, and professional patient experience. Key Responsibilities :

Room patients, obtain vital signs, and review / update medical histories

Assist with procedures and examinations under the supervision of our providers

Administer medications and injections as ordered

Provide patient education and follow-up instructions

Document clinical information accurately in the electronic health record (EHR)

Coordinate with care teams and assist with care management tasks

Perform phlebotomy and lab specimen collection (if certified / trained)

Maintain exam rooms, medical equipment, and inventory of supplies

Support telehealth visits and remote patient monitoring as needed

Qualifications :

Current and unrestricted Illinois LPN license required

Previous experience in a primary care or outpatient clinic preferred

CPR / BLS certification required

Strong communication and interpersonal skills

Detail-oriented with strong organizational and documentation abilities

EMR / EHR experience preferred (we use Epic)
