About The Position

Experience : Minimum 1 full year of Adult Home Health Experience or 1 full year of Hospice Experience

License : Must have license in hand

Certifications : CPR, BLS

Must-Have :

Proficiency in HomeCare HomeBase and Oasis

Recent experience with HomeCare HomeBase within the last 6 months

Ability to travel up to 75 miles from assigned care center

Comfortable accepting all patients on schedule regardless of location

Completion of documentation within 24 hours of patient appointments

Description :

Provides coordinates and directs the provision of home nursing care according to physician orders.

Responsible for the competent application of the nursing process based on agency policies and procedures.

Accountable to Clinical Manager.

Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses.

Home visits and periodic office meetings are required.

Must have reliable transportation, a current driver’s license, and agency-required liability insurance.

Average number of patients per day is between 5-10, equating to roughly 6-8 productivity points.

Types of cases include Start of Care, Wound Care, and Follow-up Visits.

Must be open to on-call rotations, which may include one day a week and one weekend a month.

Knowledge of patient documentation and billing procedures is essential.

All documentation must be completed within 24 hours of patient appointments.

Weekly productivity points requirement is 30, which equates to approximately 40 hours per week.

No guaranteed hours, and overtime must be pre-approved.

Opportunity to float to another nearby clinic if weekly patient numbers are insufficient for financial needs.

Local candidates are accepted.
