You will promote the use of evidence-based practice through the nursing process, protocols, and procedures. You will assess the patient's status through interview, observation, and physical assessment of actual problems and outcomes. You will develop an individualized plan of care using a collaborative approach with the patient and health care team. You will use appropriate interventions identified in the plan of care to achieve expected patient outcomes. You will systematically evaluate and document the patient's progress toward expected outcomes and goals on an ongoing basis throughout the shift. You will modify the plan of care based on evaluation and as needed based on changes in the patient's condition. You will complete required documentation as appropriate, including patient charts and event reports.
