Details :

• Setting : REMOTE (Occasional House Calls)
• Location : Remote (Virtual)
• Schedule : Full-Time (Mon-Fri 8a-5p)
• Duration : 3 months with possible extension)
• Duties :
• Maintains quality of care standards
• Assists patients with enrolling to access educational videos
• Make assessment of patients health status
• Follow-Up and evaluate patient's status
