You can find a home for your career here.\n\nAs a Hospice RN, you can expect : \n\nthe ability to develop trusting relationships as an end-of-life care expert.\n\nbeing valued and respected by patients and their families.\n\nemployee-focused wellness and support programs\n\nincredible team support and empathetic leadership\n\nTake your nursing career to a new level of caring.\n\nResponsibilities\n\nAs the RN Case Manager, you will assume full nursing responsibility for the delivery of the Plan of Care for each hospice patient, while continuously evaluating personal and professional performance and making necessary changes to increase productivity and quality of care delivered.\n\nMakes the initial nursing evaluation in determining eligibility for hospice services during visit within forty-eight (48) hours of referral.\n\nIdentifies the patient's / family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every fifteen (15) days.\n\nInitiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.\n\nDocuments problems, appropriate goals, interventions, and patient / family response to hospice care.\n\nCollaborates with the patient / family, attending physician and other members of the IDG in providing patient and family care daily.\n\nEducation And Experience\n\nExperience Requirements\n

• One year of clinical experience.\n\nLicense Requirements\n
• Current RN licensure in the state of practice and one year of clinical experience.\n
• Current CPR Certification.\n
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.\n\nAdditional State Specific Requirements\n
• MS : At least one year full-time experience as an RN.
