Physical Therapist - Outpatient Orthopedic & Musculoskeletal Rehabilitation | Elmira, NY

Key Responsibilities

• Conduct comprehensive initial patient evaluations and assessments for orthopedic, neurological, and musculoskeletal conditions
• Develop and implement individualized, evidence-based treatment plans tailored to each patient's unique rehabilitation goals
• Administer therapeutic interventions including manual therapy, therapeutic exercises, modalities (ultrasound, electrical stimulation, heat / ice therapy), and functional training
• Educate patients and families on home exercise programs, injury prevention strategies, posture correction, and self-management techniques
• Collaborate with physicians, occupational therapists, and multidisciplinary care teams to deliver comprehensive, patient-centered rehabilitation services
• Monitor and document patient progress, adjusting treatment protocols as needed to optimize functional outcomes
• Maintain accurate clinical documentation including evaluation reports, progress notes, and discharge summaries in compliance with regulatory standards
• Ensure a safe, clean, and organized treatment environment while adhering to infection control protocols and safety guidelines
