Our dedicated staff is essential in guiding patients through a continuum of care, ensuring they receive quality, medically appropriate, and cost-effective solutions that truly make a difference in their health journey.\n\nKEY RESPONSIBILITIES\n
- Conduct comprehensive assessments of members to identify eligibility for case management, utilizing clinical judgment and insight into changes in health and wellness.\n
- Engage collaboratively with members, caregivers, physicians, and healthcare professionals to develop and implement personalized case management plans that align with individual needs and goals.\n
- Perform face-to-face or home visits to build rapport and gather essential information.\n
- Monitor ongoing care plans, assessing their effectiveness and suggesting necessary adjustments to achieve desired outcomes.\n
- Maintain an active case load for consistent outreach and effective management.\n
- Advance the integration of behavioral health care and long-term services to enhance continuity of care.\n
- Facilitate interdisciplinary care team meetings to ensure a cohesive approach to patient care.\n
- Use motivational interviewing techniques to empower and support members during interactions.\n
- Identify barriers to care and provide comprehensive coordination to address concerns satisfactorily.\n
- Local travel is required, ranging from 25% to 40%.\n
- Consult and educate non-RN case managers on best practices and care strategies.\n
- Handle complex cases requiring medication reconciliation and assessment.\n\nJOB QUALIFICATIONS\n\nRequired Education\n\nGraduate from an Accredited School of Nursing; a Bachelor's Degree in Nursing is preferred.\n\nRequired Experience\n\nA minimum of 1-3 years in case management, disease management, managed care, or both medical and behavioral health settings.\n\nRequired License, Certification, Association\n\nActive, unrestricted State Registered Nursing (RN) license in good standing; must possess a valid driver's license with a clean driving record, ensuring reliable transportation for travel.\n\nPreferred Education\n\nBachelor's Degree in Nursing.\n\nPreferred Experience\n\n3-5 years of experience in case management, disease management, managed care, or medical / behavioral health settings.\n\nPreferred License, Certification, Association\n\nActive, unrestricted Certified Case Manager (CCM).\n\nMolina Healthcare values our employees and offers a competitive benefits and compensation package.
