The RN Clinical Manager / Supervisor is responsible for the overall supervision and coordination of clinical services. Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high-quality care to home health patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
Essential Functions :
Counsel's employees as needed according to Agency Policy and Procedures.Serves as resource for field staff, patients and / or families, physicians and other community and contract agencies.Obtains physical, psychosocial, environmental and health behavior assessment data as evidenced by completed admission forms.Completes all nursing documentation and forms within the timeframe as established by Agency. Notes changes in patient's condition through on-going assessment as reflected in the progress notes and follow-up assessments.Compares assessment data to baseline assessment to monitor patient's progress. Recognizes alteration in function, including life-threatening situation, and intervene and document appropriately. Initiates realistic teaching plan based on identified learning needs of the patient and / or those providing care and documents appropriately.Supervises field staff in their delivery of care as prescribed by the established Plan(s) of Care.Provides on-the-job training to field staff. Evaluates competency of field staff and provide remediation training as needed.Appropriately documents, investigates and resolves patient / employee complaints and occurrences. Provides employee health assessment and medical compliance requirements as requested.Responsible for obtaining prior authorizations as required by third party payers.Education & Experience
Current RN licensure in state of practiceCurrent CPR certification requiredCurrent Driver's License, vehicle insurance, and access to a dependable vehicle or public transportationPrior management / supervisory experience preferred.Knowledge of home care regulations and regulatory requirements is preferred.LI-SH1
