Home Health Nurse

Pride Health is hiring RN → Home Health to support our client’s medical facility based in Newton Center, Massachusetts. This contract opportunity is a great way to start working with a top-tier healthcare organization!

Job Summary :

The Home Health RN is responsible for delivering high-quality, patient-centered care in the home setting. Responsibilities include assessing patient conditions, developing and implementing individualized care plans, and providing ongoing education and support to patients and their families. Care is delivered in alignment with the organization’s philosophy, policies, and community health practice standards.

The Home Health RN also manages admission processes and fulfills case management duties in accordance with MGB Home Care policies and professional standards. This role requires thorough and timely electronic documentation, effective interdisciplinary communication, and a strong commitment to patient safety and quality outcomes.

Requirements :

Minimum 1 year of certified home care experience

Minimum 1 year of case management experience

Ability to travel to assigned patient cases

Weekend rotation required once every 6 weeks

Pay Range : $2800 / Weekly

• Offered pay rate will be based on education, experience, and healthcare credentials.
