  • Accountable for providing care that is based on licensure / certification within the scope of practice\n
  • Maintains an updated clinical record on each patient at all times, meeting required deadlines for documentation\n
  • Evaluates the effectiveness of nursing service to the patient and family on an ongoing basis\n
  • Consults with physicians concerning alterations of patient care plans, documentation of changes in patient care and reports all changes in care to the nurse supervisor\n
  • Supports FFHC's mission by mentoring and / or coaching new employees during patient specific orientations\n
  • Educates patients and their caregivers on topics related to patients' care needs\n
  • Participates in FFHC Quality Assurance, Performance Improvement program (QAPI) as requested\n
  • Continually strives to improve his / her nursing care skills\n
  • Attends and / or completes all mandatory training / in-services as assigned\n
  • Submits required personnel file documentation in a timely manner\n
  • Adheres to FFHC compliance and ethics expectations\n
  • Maintains patient and family confidentiality\n
  • Maintains professional, supportive, and responsive interpersonal communication skills\n
  • Performs other duties as assigned\n\nROLE QUALIFICATIONS\n\nEDUCATION & EXPERIENCE\n\nREQUIRED\n
  • LPN / LVN Nursing Degree. Unencumbered LPN / LVN License in the state of employment or Multi-State license through the Enhanced Nurse Licensure Compact where recognized.\n
  • 1 years of demonstrated experience\n
  • Valid driver license with proof of insurance in applicable states\n
  • Clear background screening\n
  • Working knowledge of Nurse Practice Acts\n
  • Valid and current CPR card / certificate with in person instructor led hands on skills check\n
  • Clear Hep B Screening in applicable states\n
  • Clear TB Screening in applicable states\n\nPREFERRED : \n\nSTATE SPECIFIC REQUIREMENTS : \n\nFlorida : \n\nIllinois : \n\nValid driver license with proof of insurance : Not required. Staff are prohibited from driving patients.\n\nHep B Screening : Not a prerequisite for hire. Completed as part of the onboarding process.\n\nPennsylvania : \n\nAdditional Background Screening : Child Abuse, and FBI check.\n\nMust complete TB screening with 2 step PPD, or 1 step PPD with proof of 2 step PPD administration within the last year.\n\nTexas : \n\nN / A\n\nKEY COMPETENCIES\n
  • The ability to read and communicate in English both verbally and in writing.\n\nWORKING CONDITIONS / EQUIPMENT USE\n
  • Work is performed in the patients home - not substantially exposed to adverse environmental conditions. Training may be performed in an office setting.\n
  • Must be able to lift up to thirty five pounds (35) pounds without an assisted lifting device.\n

  • This role requires travel to patients' homes to deliver care.
