Licensed Social Worker

A company is looking for a Remote Social Worker.

Key Responsibilities

Conduct comprehensive psychosocial assessments and provide grief and bereavement support to patients and families

Participate in interdisciplinary team meetings to discuss patient care plans and provide medical social work case management

Assist patients and families in accessing community resources and navigating social and healthcare systems

Required Qualifications

2+ years of healthcare social work experience

MSW in Social Work or BSW with equivalent experience

Experience in hospice, palliative care, oncology, or home health

Palliative care certification is highly preferred

Bilingual (English / Spanish) is preferred
