Title : Team Leader / Direct Support Professional

Department : Residential Programs

Location : Farmingdale, ME

Reports to : Residential Manager

Program : Residential Services

We are currently hiring a Team Leader DSP / CRMA for our Farmingdale location.

The following is an overview and not a complete list of duties that may be assigned to the Team Leader to be performed in conjunction with the Residential Manager :

• Ordering of medication / Medline / supplies
• Quality Review of all Therap data and on-site charts and manuals
• Update forms such as medical concern log’s, weekly census, purchase orders, Mileage logs
• Filing / purge paperwork
• Assist with payroll and scheduling to include data collection of staffing patterns
• Primary Trainer of DSP’s at their site as their schedule allows
• Assist with personal spending and personal finance of people support
• Doctor appointments and prepare consultation forms via Therap
• PCP meeting
• Facilitate fire drills and complete associated paperwork
• Menus and grocery shopping as well as overseeing inventory following purchase
• Fire safety and extinguishers and emergency exit light checks
• Reviewing MARS for Quality Assurance and Preregister for appointments
• Monthly house generator checks and documentation
• ISP data review and monthly summaries completed and printed
• T-Logs checked for Quality Assurance daily and / or weekly
• Quarterly Reports and Monthly Medical Summaries completed each quarter
• Scanning of Medical information into Therap
• Updating Personal Property inventories in Therap

Pay rate depending on experience starting at $18.12 an hour.

Must have a valid State of Maine Driver's license

Must have a good driving record with active auto Insurance.

Must have Reliable Transportation

Must be 18 years of age or over

Education : High School diploma or GED. CRMA required.
