Licensed Pharmacist - Cost Containment

Responsibilities :

• Utilizing Excel to Analyze high-cost medication orders and identify cost-saving opportunities through therapeutic interchange while ensuring patient safety.
• Track missed automatic therapeutic interchange prescriptions and fix and accept T / I accordingly.
• Review medication profiles to verify the appropriate drug selection, dosage, interactions, and identify potential errors.
• Recommend therapy changes to physicians and facility staff.
• Communicate efficiently with nursing homes via phone, fax, and email regarding therapeutic interchange recommendations and high-cost order clarifications.
• Apply clinical knowledge of medical history, current therapies, and laboratory data to support decision making.
• Perform additional tasks as assigned by the Cost Containment Supervisor.
• Demonstrate initiative and interest in expanding knowledge of various pharmacy practices.
