Physical Therapist - Lexington, MA

Job type : Full-Time (Onsite)

Setting : Assisted Living & Memory Care Facility (Not Outpatient)

Schedule : Monday-Friday (On-call requirements discussed during interview)

Job Overview

We are seeking a dedicated and compassionate Physical Therapist to provide personalized, one-on-one therapy services to residents in assisted living and memory care communities. This role offers the opportunity to make a meaningful impact on seniors' lives in a supportive and collaborative environment.

Key Roles & Responsibilities

Evaluate patients to assess mobility, strength, balance, and functional limitations.

Develop and implement individualized treatment plans to improve mobility and reduce pain.

Provide interventions such as therapeutic exercise, manual therapy, gait and balance training.

Promote safety and fall prevention through proactive assessments.

Educate patients and families on home exercise and independence strategies.

Document patient care in accordance with state, facility, and EMR standards.

Collaborate with interdisciplinary teams and participate in professional development.

Qualifications

Graduate of an accredited Physical Therapy program.

Active or pending Massachusetts PT license.

Strong interpersonal and communication skills.

Commitment to patient-centered, evidence-based care.

Experience or interest in working with the senior population.
