Job Description

Your Impact:

• Deliver Field-Based Care: Conduct comprehensive primary care visits and physical exams in the comfort of a patient's home.
• Manage Chronic Disease: Create longitudinal care plans that move the needle for high-risk patients.
• Collaborate Across the Continuum: Partner with an interdisciplinary team to ensure seamless, safe care transitions.
• Prevent ER Visits: Treat acute needs in the home to keep seniors safe, healthy, and out of the hospital.

Job Requirements

What You Bring:

• Advanced Degree: Minimum Master's Degree in Physician Assistant Studies (ARC-PA) from an accredited, entry-level PA program (required).
• New York Credentials: Active, unencumbered Physician Assistant license in the state; state residency required. Current board certification in your field with NCCPA (National Commission on Certification of Physician Assistants) (required).
• Reliable Mobility: Valid driver's license and vehicle for local field-based travel.
