Job descriptionCoordination : Manages delivery of meals, DME, and caregiver services. Documentation : Tracks actions and updates participant records. Communication : Regularly interacts with participants, families, and healthcare providers. Support : Assists senior care nurses and collaborates with the interdisciplinary team. Certificates : Must have a Home Health Aide certificate and CNA certificate. Experience : At least one year of experience with the elderly population. Skills : Basic computer skills and ability to work in a team environment. Meetings : Attends staff meetings and training programs. Visits : Conducts home visits and assists with surveys. Compliance : Adheres to health and safety standards and organizational policies.
Position Summary :
- Responsibilities : Conducts home safety evaluations, coordinates meal delivery, caregiver services, and durable medical equipment (DME) for participants and provides support to the Home Care team.
- Advocacy : Ensures care is delivered in a person-centered and compassionate manner.
- Hours
Monday-Friday 8 : 00am-5 : 00pm
