Position : Physical Therapist (PT)

Shift : Day Shift (8 : 00 AM - 4 : 30 PM)

Shift Length : 8 hours

Contract Duration : 13 weeks

Role Overview

The Physical Therapist (PT) will provide high-quality rehabilitative care to residents in a skilled nursing or rehab setting. The clinician will assess functional deficits, develop treatment plans, and deliver therapy services aimed at improving mobility, independence, and overall quality of life.

Key Responsibilities

• Conduct evaluations to assess patient mobility, functional limitations, and rehabilitation needs.
• Develop individualized treatment plans based on clinical evaluation and patient goals.
• Deliver therapeutic interventions including gait training, therapeutic exercise, balance training, and functional mobility.
• Document assessments, treatments, and progress notes in accordance with clinical and regulatory standards.
• Collaborate with occupational therapists, speech therapists, nursing staff, and the interdisciplinary care team.
• Provide education to patients, families, and caregivers regarding exercises, safety, and functional improvement.
• Participate in care plan meetings and discharge planning as needed.
• Maintain compliance with all facility policies, state regulations, and professional standards.
