Role Overview
The Physical Therapist (PT) will provide high-quality rehabilitative care to residents in a skilled nursing or rehab setting. The clinician will assess functional deficits, develop treatment plans, and deliver therapy services aimed at improving mobility, independence, and overall quality of life.
Key Responsibilities
- Conduct evaluations to assess patient mobility, functional limitations, and rehabilitation needs.
- Develop individualized treatment plans based on clinical evaluation and patient goals.
- Deliver therapeutic interventions including gait training, therapeutic exercise, balance training, and functional mobility.
- Document assessments, treatments, and progress notes in accordance with clinical and regulatory standards.
- Collaborate with occupational therapists, speech therapists, nursing staff, and the interdisciplinary care team.
- Provide education to patients, families, and caregivers regarding exercises, safety, and functional improvement.
- Participate in care plan meetings and discharge planning as needed.
- Maintain compliance with all facility policies, state regulations, and professional standards.
Qualifications
Active state Physical Therapist license.Experience in skilled nursing or rehabilitation settings preferred.Strong clinical assessment and documentation skills.Ability to work independently while collaborating effectively with the care team.Excellent communication and patient-interaction skills.
