Social Worker - Case Manager

Opportunity Description Immediate need for an experienced Social Worker to support multiple facilities in the Syracuse NY area. Job Duties

• Assigned to provide coverage as needed at multiple facilities
• Provides comprehensive psycho-social assessment, intervention, management, support, and assess for discharge planning and aftercare needs of the at-risk patient population and their families.
• Addresses social determinants of health and works collaboratively with the multidisciplinary team.

Experience & Skills Required

• 3 years experiencen in a hospital or healthcare setting
• Masters Degree
