Community Support Advocate - Full Time

The Community Supports Advocate works as a member of the Family Advocacy and Community Education team to provide opportunities for community integration for adults with an acquired brain injury to increase individual participation within their local community, develop social connections, reduce social isolation, and strengthen leisure options and social skills. The Advocate will work with individuals and their caregivers to establish goals for participation in the community. The Advocate will identify and recruit partner organizations to assist in meeting identified needs.
ESSENTIAL FUNCTIONS :
- Assist in developing a monthly calendar of small group and individual community outings and center-based activities for adults with acquired brain injuries.
- Present creative, innovative ideas that will increase opportunities for individuals with brain injuries to participate in recreational services in their own communities.
- Individualize the program to meet goals of participants and track progress.
- Develop a working knowledge of programs and activities offered in the community.
- Identify potentially underserved and under-supported community members and provide recreation program promotion and outreach to these groups to expand recreation services access and participation.
- Introduce individuals and their family members and friends to a network of other supports and services offered to people with brain injuries.
- Develop a consistent intake process for individuals participating in the program.
- Assist in planning and implementation for Community Celebrations events.
- Assist the program coordinator in other community support events, including those outside the MassAbility ABI Recreation program
- Develop partnerships with community organizations in order to enhance services available to individuals and their families.
- Coordinate workshops and special activities.
- Develop and distribute program materials and literature to inform individuals, caregivers, and the community of program activities
- Provide information for and attend ISP meetings when needed.
- Participate as a member of the Family Support Center team.
- Participate actively in program training, in-service training sessions, program meetings, supervision, and program evaluation.
- Complete and maintain documentation according to program specifications.
- Respond to program emergencies as needed.
- Attend meetings, trainings, and professional development as required.
- Maintain all program and Agency required trainings.
- Perform other duties as assigned by management.
QUALIFICATION :
High school diploma or equivalent and minimum of three years' experience in human servicesKnowledgeKnowledge of acquired brain injury services preferredKnowledge of family support concepts and practicesKnowledge of local community-based resources for children and familiesKnowledge of and respect for cultural diversity.Skill in communicating orally and in writingSkill in building relationshipsAbility to work independently and as part of a teamAbility to participate in and undergo extensive orientation, in-service training, and supervisory observationAbility to maintain a valid driver's license and reliable transportationAbility to work effectively with children and adults with a developmental disabilityA qualifying criminal background check and driving record and a valid driver's license are required.Ability to provide current copy of TB assessments, and then a copy every two years.Why join our Team?
