Physical Therapist – SNF

Location : Hillsdale, MI

Schedule : Full-time

Setting : Skilled Nursing Facility (SNF)

Overview :

We’re seeking a dedicated Physical Therapist (PT) to join a skilled nursing facility team in Hillsdale, MI . This role offers the opportunity to provide patient-centered care aimed at improving mobility, function, and overall quality of life for residents.

Responsibilities :

Deliver physical therapy evaluations and treatments in the skilled nursing setting

Develop and implement individualized care plans based on patient needs

Collaborate closely with interdisciplinary team members to coordinate care

Maintain accurate and timely documentation in compliance with state and facility requirements

Educate patients and families on therapy goals and home exercises

Qualifications :

Active Michigan Physical Therapist license

Experience in skilled nursing or geriatric care preferred

Strong clinical skills and effective communication abilities

Ability to work independently and collaboratively within a team
