Overview

We are seeking a dedicated and compassionate Home Health Registered Nurse (RN) Case Manager to join our care team in Irwin, Pennsylvania. This full-time position offers competitive pay, comprehensive benefits, and a supportive, team-oriented environment. The RN Case Manager will lead and collaborate with an interdisciplinary team to manage and deliver personalized patient care plans for individuals receiving care at home.

Key Responsibilities

Provide leadership in the management and delivery of patient care plans

Support patients in Irwin and surrounding areas

Collaborate with the care team to ensure continuity and quality of care

Meet critical charting and documentation deadlines

Effectively self-manage time and daily schedule

Utilize EMR systems, tablets, and mobile technology for charting and communication

Schedule and Shift Details

Monday through Friday, 8-hour days

One evening of on-call per week (5 : 00 p.m. 10 : 00 p.m.)

One weekend per month (Saturday / Sunday)

Qualifications

Required

Associate degree or higher in Nursing

Current Registered Nurse (RN) license in Pennsylvania

Minimum of 1 year of clinical nursing experience

Proficiency with Electronic Medical Record (EMR) systems such as TruChart EHR or similar

Valid driver's license and reliable transportation

Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications from the American Heart Association

Willingness to travel within the service area as required

Preferred

Previous experience in home health or case management

RN license number provided upon application
