Clinical Social Worker - CSWA / LCSW (Palliative Care)

The Clinical Social Worker position provides biopsychosocial assessments / interventions, crisis assessment / interventions, care coordination, referral coordination, and advocacy for patients and their support system(s) as a member of the interdisciplinary team for Lower Columbia Hospice. This position oversees the bereavement program to support loved ones of hospice patients as well as providing grief support to the community through education and support groups, often in partnership with trained volunteers. This position is also supported by connection to the larger group of Social Worker and RN Care Coordinators in the Care Management department of Columbia Memorial Hospital.

Physical Demands and Work Environment Conditions / ADA Compliance Often work alone; travel to patient's home and other health care institutions-must drive or provide own transportation; normal or corrected vision is required-normal or corrected hearing is necessary; physical and emotional stress of dealing with death and dying. Position involves standing, walking, stooping, and occasional carrying and lifting of lightweight materials (up to 40 pounds). May be required to sit for extended periods of time. Requires visual acuity and depth perception to recognize words and numbers; and hand and finger dexterity and hand / eye coordination to use a computer keyboard and common office equipment.
