Illinois Care Management Coordinator

A company is looking for a Care Management Support Coordinator II to assist with care management activities remotely in Illinois.

Key Responsibilities

Conduct outreach to members to support care plan next steps and connect them with resources

Serve as the front-line support for member and provider inquiries regarding care plan procedures

Document and maintain non-clinical member records in compliance with state and regulatory requirements

Required Qualifications

High School diploma or GED required

1 - 2 years of related experience in a healthcare setting

Familiarity with medical terminology

Knowledge of health plan activities and community resources

Must reside in the State of Illinois
