Position Overview

We are currently hiring a Registered Nurse (RN) to perform skilled nursing visits for our home health patients. The ideal candidate will have strong clinical judgment, excellent communication skills, and a caring attitude toward patient-centered care.

Responsibilities :

Perform comprehensive Home Health Assessments for current patients.

Assess, plan, implement, and evaluate individualized care plans.

Document all visits, findings, and interventions accurately and in a timely manner.

Educate patients and caregivers about medication, treatment plans, and safety practices.

Coordinate with physicians, case managers, and other healthcare professionals.

Ensure all care meets regulatory and agency standards.

Qualifications :

Active Texas RN license (unencumbered).

2+ years of clinical experience (1+ year home health experience REQUIRED).

Excellent communication and organizational skills.

Reliable transportation and valid driver’s license.

Compassionate and dependable with a strong sense of integrity.

Schedule & Compensation :

Flexible schedule (PRN).

Competitive pay ($90 per visit or $125 per initial assessment).

Supportive and collaborative work environment.

Location :

Travel required throughout the Houston metro area for in-home patient visits.
