Job Overview

The CHW Program Coordinator is responsible for identifying high-risk patients for poor health outcomes. The role also coordinates program activities, analyzes data, and organizes/coordinates CHWs caseloads. Role will support data collection and support overall program evaluation. Our organization offers an innovative, evidence-based health care program that improves health outcomes for low-income, high-risk patients using Community Health Workers (CHWs). In this model, CHWs provide structured advocacy, social support, and navigation to high-risk patients in order to help them reach their health goals.

Minimum Requirements

1. High School Diploma required.

2. Minimum of 3 year experience with data collection, management, and/or analysis.

3. Minimum of 2 year experience extracting information and exporting data to other applications.

4. Experience with Epic EMR system a plus.
