Nurse Practitioner - Subacute/ Acute Care, Hospital Rounding & Skilled Nursing

We are seeking a dedicated and highly skilled Nurse Practitioner (NP) to join our healthcare team. The ideal candidate will have strong experience in subacute and acute care, with the ability to diagnose and treat medically complex patients across multiple care environments. This position functions as a key clinical provider within a skilled nursing facility that includes both long-term care residents and a specialized ventilator/tracheostomy unit, as well as performing hospital rounds in collaboration with hospitalists.

The NP provides comprehensive medical management, daily rounding, acute issue response, and interdisciplinary care coordination to ensure high-quality patient outcomes. This role requires strong assessment skills, independent clinical decision-making, and the ability to manage complex patient populations in both facility-based and hospital settings.

This is a five-day flexible schedule that includes one weekend day as part of the regular weekly schedule.
