Position Profile

The Dementia Education and Partnerships Director serves as both educator and ambassador, bridging Hospice of the Valley’s mission with the communities it serves. This role integrates educational leadership, community partnership development, and mentorship to advance comfort, dignity, and compassionate dementia care. The Director will expand educational collaborations with assisted living communities and Arizona-based employers while strengthening internal and external alignment in dementia education and practice.

Responsibilities

  • Strategic Partnership Development.
  • Corporate and Community Outreach Initiatives.
  • Program Design and Inventory.
  • Mentorship & Staff Development.
  • Program Evaluation & Reporting.
  • Maintains and enhances professional skills.
  • Demonstrated ability to promote patient-centered care.
  • Adheres to scope of practice / HOV standards and facilitates continuously improved processes / services.
  • Minimum qualifications

  • Masters in Social Work, Education, Healthcare Administration, or a related field.
  • Minimum five years leadership experience.
  • Minimum five years in dementia care across multiple settings.
  • Demonstrated expertise in adult education, curriculum design, facilitation, and mentorship.
  • MSW or RN preferred.
