The New Jersey State licensed LAC or LSW collaborates with fellow treatment team members to ensure the provision of quality care tailored to the unique needs and care levels of each patient within an outpatient treatment setting They are responsible for delivering direct services to patients and their families enrolled in PAX Health, including individual, group, and family therapy sessions.
Responsibilities include but are not limited to :
- Receive, review, and promptly respond to intake referrals upon assignment of the case.
- Conduct Bio / Psych / Social assessments diagnostic impression and formulate treatment plans under the guidance of the Clinical Director.
- Meet for individual sessions with patients at a frequency determined by the ongoing assessment of progress in treatment.
- Complete and assist clients in aftercare and discharge planning
- Provide direct clinical services, including individuals group, family and / or couples therapy to clients and their families.
- Provide case management services as needed, including referrals to outside resources when necessary
- Ensures cohesion of all services documented within the clinical chart (including but not limited to services provided by case management medical providers and the laboratory), and assists with compliance of various standards.
- Maintain accurate, thoroughly documented client records with a quality of documentation that meets all applicable standards in the Electronic Health Record (EHR).
- Utilize EHR to input all clinical documentation
- Attend and participate in treatment team meetings as scheduled to ensure quality of care for clients
- Attend staff development and general staff meetings
- Attend staff and clinical trainings
- Communicate with Medical Director and other prescribers to ensure quality clinical services are provided to the client on a timely and as needed basis Participate in and attend supervision / consultation with Clinical Director.
- Collaborate with members of the multi-disciplinary treatment team to ensure effective delivery of a full spectrum of clinical services
- Comply with local, state and federal confidentiality laws and regulations
- Collaborate with professionals both within and outside the agency, as appropriate
- Demonstrate sensitivity and responsiveness to cultural differences
- Abide by PAX Health code of ethics and model values
Requirements : Qualifications :
If you are seeking a part-time position, we ask for a minimum dedication of 20 hours per week.Must be able to communicate with patients and staff members through various forms of communication, including email, phoneComputer literacy is required. Must be able to use Word, Excel, etc. Experience on Charm is a plusMasters in Social Work, Psychology or related field AND :Current license (NJ) to practice as LSW, LAC working toward above licenses under direct supervision of an approved supervisor as per state regulations .LCADC or CADC is a plus.
